Bunionette (Tailors) Toe Deformity

Ankle and foot anatomy

The foot consists of the Hindfoot (calcaneus and talus bones), Midfoot (navicular, cuboid, cuneiform bones) and Forefoot (metatarsal and phalanx bones).

The ankle joint is a hinge joint with muscles crossing it to allow ankle range of motion (ROM) while muscles crossing the midfoot and forefoot area allow foot ROM.

Bunionette 5th toe deformity

  • Bunionette (Tailors) little toe deformity is prominence of the lateral aspect of the metatarsal head area
Clinical presentation:
  • Local pain
  • Bursa formation or ulceration
  • Pain aggravated by wearing tight fitting shoes

Classification

Type:
  1. 1: Enlarged metatarsal head
  2. 2: Lateral bowing of metatarsal head
  3. 3: Increased lateral bowing with increased intermetatarsal angle between 4th and 5th metatarsal (IMA > 8°)

Tailors bunion management

  • Non operative measures include analgesia, shoe wear modifications with wider toe box and padding over the lateral bunion area
  • Definitive management is surgical correction of the deformity
