Infrapatellar tendon problems

Patellar tendinopathy is inflammation of the Infrapatellar tendon and it is usually associated with activities with repetitive forceful quadriceps contraction.

It presents with localized pain and tenderness near the patellar tendon insertion at the inferior pole of the patellar.

Infrapatellar tendon problems

Infrapatellar tendinosis management

Management is usually conservative
  • Modify aggravating activities while undergoing treatment and rehabilitation
  • Analgesia
  • Platelet injection is a consideration
  • Dry Needling treatment modality is a consideration
  • Rehabilitation focusing on extensor muscle strengthening
  • Maintain fitness with upper limb ergometry while rehabilitating

Ruptured Infrapatellar tendon management

Surgery
  • Complete, acute rupture requires surgical repair
  • Additional lengthening/graft augmentation/reconstruction procedures may be required for chronic tears

V-Y lengthening procedure:
  • Make a V incision
  • Lengthen the quadriceps tendon
  • Closure of the distal aspect of the V side to side
